A tram driver reportedly fell asleep, causing the vehicle to jolt violently and startle passengers in San Francisco.
Footage shows the driver nodding off, tilting her head to the side while seated in the control cabin on September 24.
Moments later, the vehicle shook abruptly, prompting the driver to react quickly and regain control.
Another clip shows passengers visibly shaken and tumbling onto their seats around 8:37 am.
The driver immediately said: 'I'm sorry, relax, relax, relax. It wouldn't stop. We didn't crash.'
Officials from the San Francisco Municipal Transportation Agency (SFMTA) said the incident occurred when the two-car N-Judah train was inbound towards downtown San Francisco.
The agency added that the vehicle experienced a 'series of sudden jolts' while navigating a curve at an excessive speed of 50 mph as it exited the Sunset Tunnel near Duboce Park.
They confirmed on Monday that the cause was 'operator fatigue'. The driver has been placed on non-driving status following the incident.
SFMTA Director Julie Kirschbaum said in a statement: 'Safety is always our top priority. We are committed to accountability in response to this specific unacceptable incident, and we are taking all necessary steps to keep Muni safe and reliable for all riders and the public.'
